HPV in men: carriage, sperm, and fertility
HPV is not a women's disease. The virus also affects men and can reinfect women through sperm. This is not about blame, but about tackling the virus together.
No question of blame, but a shared approach
When a woman tests HPV-positive, tension often arises in the relationship. Where did it come from? Who infected whom? These questions are understandable, but rarely productive. HPV can lay dormant for years before it is noticed. The virus could have been contracted from a previous partner, sometimes years ago.
At HPV Health, we believe it's not about the question of blame, but about the solution. As a couple, you can work together on clearing the virus. That begins with understanding: HPV is a joint problem that requires a joint approach.
HPV in sperm: what does that mean?
of sperm samples with normal parameters contained HPV. More than 1 in 5 men with apparently healthy sperm carry the virus.
HPV can be present in a man's sperm. This is not just a side note: the virus in sperm is infectious to the woman. During ejaculation, the sperm comes into direct contact with the cervix, exactly the place where HPV can cause the most damage.
This means that even if a woman has been successfully treated for HPV, infection via her partner remains possible, especially if the immune response against that specific type of HPV has not yet fully developed. Infection with other HPV types is also possible. In men, just as in women, a large portion of men will clear the virus, but carriage of HPV viruses can also occur. It is a very contagious virus because it is transmitted quickly after sexual contact. This is why the vaccination is also offered to boys.
HPV and fertility
clinical pregnancies in women who were inseminated with HPV-positive sperm, compared to HPV-negative sperm.
The impact of HPV on male fertility is often underestimated. An HPV infection in the man can affect several aspects of sperm quality:
- DNA integrity of the sperm is affected
- Motility decreases
- Sperm count may decrease
- Viability is negatively affected
- Morphology (shape) can be abnormal
- HPV can induce anti-sperm antibodies
For couples wishing to conceive, it is therefore particularly relevant to also test the man for HPV. A normal sperm test does not rule out the presence of HPV.
When does HPV remain present longer?
Just as with women, a man's immune system can usually clear the HPV virus on its own. But in certain men, HPV can remain present longer. Factors that play a role in this include a weakened immune system, smoking, and co-infections.
HPV infections can be accompanied by other infections such as chlamydia and others. Co-infections make it more difficult to clear HPV.
Low-risk HPV: genital warts
HPV types 6 and 11 are low-risk types that can cause genital warts (condylomata) in men. These warts are visible on the penis, the scrotum, or around the anus. Although they do not cause cancer, they are contagious and can cause a new HPV infection in the woman.
It is important to realize that the virus can be present even without visible warts. Many men carry HPV without any complaints or symptoms.
HPV test for men
At HPV Health, we offer a specific HPV test for men. We test both the sperm and the penis for the presence of the HPV virus. In addition, we also test for trichomonas vaginalis, a common co-infection.

Action plan for men
Get tested
The first step is knowing if you carry HPV. A sperm and penis test at HPV Health provides clarity. Without a test, you don't know if you are carrying the virus.
Stop smoking
Smoking weakens the immune system and makes it harder for your body to clear HPV. Quitting smoking is one of the most impactful steps.
Strengthen your immune system
Good nutrition, sufficient sleep, stress reduction, and targeted supplements help your body tackle the virus.
Treat co-infections
Attention to accompanying genital infections is important.
Work together as a couple
HPV is a joint problem. Discuss the situation openly with your partner and tackle it together. This prevents reinfection and strengthens your chances of recovery.
